Recently I've spoken to two different Doctors who told me that surgical intervention at birth to correct "gender" irregularities is increasingly being abandoned. When possible, all things being equal, parents are being strongly encouraged to give the child a role in that decision at a much later age.
Here in America, this has been a long hard battle.
On another note, there is now discussion regarding combining Canadian and American Military into one on certain "police" actions. I would agree with this as long as the Canadians are put in charge.

it is doubtful that the military would be combined on deployment, the "friendly rivalry" between the two countries armed forces often gets pretty heated. ;)
and there is absolutely no way the arrogant us military would accept any other country being in command of them in ANY action. Even we Canadians do not have full knowledge of the capabilities of the US forces, so they couldn't be effectively utilized.

While I don't think that it has been done in large scale since WWII, members of the US Military are fairly regularly put under the command of allied countries command.
The invasion of Libya a few back was a French operation that the US participated in.
