I wondered that myself. Had a nice comment about that after pt. 3, but ff has been screwing up since the last update and closing tabs when I clicked on them to open them.
This is everything I hate about shrinks. Nobody appointed you god, buddy. I'll go to hell in wharever way I choose, so butt out. I would be pissed, really pissed with Meghan.

Psychiatrists
don't command the pay of a lot of other doctors (surgeons, specialists) but they wield soooo much power. If they label you as bipolar, or suicidal. or schizophrenic, or even just plain ol' major depression - that label sticks for life, and can affect your future (as in employment, etc.). Like anything else, there are good ones and bad ones; the key is to find a good one. Easier said than done. (Gee, you might almost think I was speaking from experience? Yes, but not personal experience; family and friends)

Yes! Antidepressants! For 6 month...
Does anyone ever read antidepressants leaflet? About every one of them lists suicide as one of the side effects! And quite a few have depression as a side effect...
And most of the time half liter of vodka per day for a week works much better than 6 months of "antidepressant". And after a week you could go "cold turkey" without any side effects! (And yes, I've seen several people who tried suicide through drinking and smoking... So far all of them survived until the day when they found a new reason to live and stop drinking and smoking.)
What I read
says that the reason antidepressant meds list suicide as a side effect is this: severe depression can make you suicidal, but also destroys your energy and motivation, so that you don't have the get-up-and-go to even perform a good suicide. When you start an anti depression medicine, the first thing that improves is energy and motivation, but NOT the suicidal thoughts yet! So you still have the death wish but NOW have the energy to carry it out. This is a temporary phase, as once the meds kick in fully (It can take weeks to months?) then the suicide tendency goes away, usually.

I really hope you're right. In fact, if this is correct, all prescribers should be informed of this, and suicide-avoidance done when it's needed.

It is not _that_ simple. And about 6.5 billion people currently successfully survive without any kind of access to prescription antidepressants! And about 3 billion even have no access to vodka of any kind!.. And still... They survive!

Walking wounded
Rick went into a one sided marriage, where he loved his wife but she didn't love him enough to accept him with all of his limitations. She felt her needs were more important than their marriage, the very thing which caused Rick to almost end it all over a very selfish prat.
But why can't Rick perform to her satisfaction? Is it a decision he made long ago, a promise to someone? An injury? Or a better question is why doesn't he want his family involved? Is their business illegal or something else Rick wants nothing to do with?
As this story advances more questions arise than answered.
Others have feelings too.

Deep story
I really don't want a nose tweak.
This story is doing a good job delaying the reveal of what Rick's problem is that his wife couldn't accept. I'd think that if he's somehow physically unable to perform sexually he would have mentioned it to the psychiatrist when they were alone since it was the driving force of his separation, and the doctor seems to think sex with Meghan is a possibility.

Rick did mention it to the psychiatrist, but the psych doc isn't going to reveal it to Meg or anybody unless Rick gives him permission to; I think Rick's too embarrassed about it to reveal it to Meg yet.
